Minister's Affirmation

"I (Minister's name), do hereby declare that I believe and embrace the Fundamental Doctrine as stated in the Articles of Faith as set forth in the Manual of the United Pentecostal Church International. I also believe and embrace the holiness standards of the United Pentecostal Church International as set forth in said Articles of Faith, and I pledge to practice, preach, and teach the same.

Along with the statement is supposed to be sent the preamble (which is partly quoted on the previous page).  These forms are to be sent 60 days prior to the expiration of the current fellowship cards.

If the minister fails to sign or return the statement within this 60 day period, they are to be notified that within 30 days their membership card will be suspended. After this 30 day grace period, the suspended minister would have to reapply for a license, appear before the District Board, and sign the affirmation statement before they could be reinstated. In other words, sign the form or lose your license.
